Teplis Travel maintains a computerized database of all our corporate travelers. This information insures that we book your travel exactly the way you want it. Included in our database are your Airline Frequent Flyer numbers, your Hotel & Car Rental Frequent Customer numbers, and your preferences regarding seat assignments hotel accommodations, and rental car options. We also like to have your home contact information in the event a flight is cancelled or we need to contact you regarding your reservation during non-business hours.

If this is an updated profile simply enter your name and company name and the information that has changed. It is not necessary to reenter all your information. Please check the “updated profile” box at the bottom of the form if you are submitting updated information.

    The airlines are now required to submit the passenger’s full name (First, Middle, Last) exactly as it appears on Government issued documentation, date of birth and gender to the TSA before boarding passes can be issued.
